What it had to solve
Heart disease is among India's biggest killers and early detection dramatically reduces the risk of fatality, but most Indians see a doctor only after symptoms appear. ACKO, a digital insurer, wanted to start a preventive heart-health conversation and used World Health Organization thresholds — waist-to-hip ratio above 0.85 for women or 0.90 for men — as its medically grounded cue.
How it works
Heart disease is among India's biggest killers, yet most people only see a doctor after symptoms appear, even though detecting risk early dramatically reduces the chance of fatality. ACKO, a digital insurer, wanted to start that conversation on World Heart Day 2024 — and it found its channel in an unexpected place: the local tailor.
Tailors measure every customer's waist and hips, and those two numbers form the waist-to-hip ratio, which the World Health Organization treats as an early indicator of cardiovascular risk — above 0.85 for women and 0.90 for men. The data needed for a health nudge was already being collected at every fitting.
ACKO turned that routine into 'The Tailor Test'. Partnering with tailor welfare associations across India to reach over one million tailors, the company ran workshops and handed out WHO-approved charts and specially designed billing books, so a tailor could compare the measurements, quietly flag an at-risk customer and point them to a free check-up helpline — no clinic, no lecture.
The intervention produced measurable behaviour change: over 50,000 calls to the free heart check-up helpline, 648,000+ branded searches and 424,904 health insurance quotes. The campaign won a Gold Lion in Creative Data and a Bronze in Brand Experience & Activation at Cannes Lions 2025, then the Direct Grand Prix at Spikes Asia 2026.

D&AD archived it as a reference for turning hidden, everyday data into a street-level health intervention.
